Superb Overview of the Topic
This text is one of a series focused on early diagnosis and treatment. This is not a definitive book on prostate cancer, it does not contain the details one would see in DeVita et al. Yet it covers in detail every aspect of the disease with a true focus on dealing with those cases where early detection has been achieved.
Chapter 1 is a good overview regarding the markers; PSA, PSA velocity, age factors and the like. The recent debate regarding the PSA stats in NEJM for example is refocused by reading their Chapter 1. Lowering PSA for younger men and focusing on velocity and free % numbers assists in the initial steps.
Chapter 2 is focused on the biopsy and staging issues. It is wonderfully presented in that it provides details and graphic materials that set the current practice in the collection and processing of the samples. One will not walk away becoming expert on Gleason scoring yet one can be brought up to date quickly. For the general practice physician the material will greatly assist in informing their patients and for the educated patient facing the problem of a diagnosis it prepares them for what to expect. The fear of the unknown on the part of the patient is often the most unsettling. This book can and the authors stats as such be used by well educated patients and hopefully avoid the many false rumors that seem to spread.
Chapter 3 is histology and the details on Gleason. This is clearly helpful for the general practitioner in working with their patient. It is one of the best discussions of Gleason scoring that I have seen. The pictures are all spot on and well discussed and they are all in color as one would seem them in a good path report.
Chapter 4 deals with the watchful waiting issue. This is the chapter on what will happen statistically if one does nothing. The problem of course is that every patient is different and there is no way to determine outcome if one just waits. Prostate cancer is one of those difficult cancers wherein 80-90% of the patients may very well never die or have any significant morbidity from the disease and yet the remaining may die in a very short period of time. This is a genetic problem, understanding what pathways are activated or not. To data there are no adequate tests to determine that subgroup.
Chapters 5 thru 7 describe the surgical procedures. These are also superbly presented. They detail the procedures, then they carefully go through the risks and success of each. The detailing of the morbidities are critical since the patient must be made aware of them in the selection process. Ultimately it is the patient's decision. Yet, that decision may be de facto made depending on the physician that he goes to. Yet educating the family physician as well as the patient is essential.
Chapter 8 deals with the various radiation modalities. Again the text provides a superb discussion of the techniques and how they are employed as well as providing details on the morbidities.
To the patient and the family physician the use of Chapters 5 thru 8 allow for a well informed method of choosing the final procedure.
Chapters 10 and 11 discuss alternatives such as cryotherapy and alternative medicine. Finally Chapter 12 goes over the many controversies. For the patient and the family physician this is a useful chapter since there is a wealth of information floating around that may not always have any foundation in truth.
All in all this is a fantastic contribution to the field. For the educated it is read in just a few hours, skipping what may be well known but seeing how the author has pulled together all of the pieces. It is worth a place in many a library. In addtion the on line data is quite useful as well.
